The Chronic Disease programme is for people who are diagnosed with one or more of the following conditions:
– Type II Diabetes
– Asthma
– Chronic Obstructive Pulmonary Disease
– Cardiovascular Disease (Heart Failure, Angina, Stroke, Irregular Heartbeat)
To be eligible for this scheme you must be aged 18 or older and have a valid General Medical Card (GMS Card) or Doctor’s Visitor Card (DVC).
The programme includes two free reviews in every 12-month period that you are registered for the programme. Each review includes one visit with the practice nurse followed by a visit with your GP. After each review you will receive a written care plan which outlines the steps that you need to take to manage your condition and the supports available to you. It can include your concerns, agreed goals between your reviews and helpful advice.
